We are always aware that its multidisciplinary base is both the strength and weakness of the Journal of Clinical Pathology. Although some papers represent the work of a single discipline1–3 we encourage submissions that have multispecialist involvement.4,5

We are pleased to include our first Grand Round Presentation in this issue. We hope that this section will help to answer the educational requests of trainee pathologists, but still be of interest to more senior readers.

The patient presented is a 48 year old man whose initial problems may be classified as haematological. Shortly afterwards he developed signs and symptoms consistent with pronounced biochemical abnormalities, and the underlying cause is explained by the histopathological changes in his kidneys. The investigation and management of the patient enable the authors to address important issues, which at present are within the remit of the specialist hospital clinician.
