      Only occasionally did the pathologist receive a surgical specimen – usually with advanced carcinoma, which, however, was then often misdiagnosed as carcinoma of the cardia. Since the beginning of the 1990s, however, western countries experienced a marked increase in the incidence of these carcinomas, which prompted the introduction of the term ”new epidemic“.1 This then led to an enormous improvement in the endoscopic-bioptic diagnosis of the neoplasias in Barrett's oesophagus, with the result that numerous neoplasias can now be diagnosed as low-grade intraepithelial neoplasia (LGIEN), high-grade intraepithelial neoplasia (HGIEN), or early carcinoma limited to the mucosa.
