CT appearances in macronodular hepatosplenic tuberculosis: A review with five additional new cases

Abstract

Pseudotumoral or macronodular hepatosplenic tuberculosis (HSTB) is rare. Only 31 cases have been documented in imaging literature so far. Presented is the clinico-imaging review with five additional new cases of this uncommon variety. Due to nonspecific wide spectrum of imaging appearances, biopsy is mandatory in almost all cases. Clinical recovery and resolution of lesions on imaging may not be directly proportional.

      In several large series of patients with post-primary tuberculosis, no mass-like opacities were seen.2 From our search we identified 1389 citations out of which 72 were related to tuberculosis complicating bronchogenic carcinoma; 13 citations related to pseudotumoral forms at other sites17–29 and 18 related to pseudotumoral pulmonary tuberculosis. Of the 18 references, only 6 citations (including 27 cases) reported parenchymal pseudotumoral tuberculosis3,6–10 (Tables 1 and 2).

      This type is also accompanied by splenomegaly [1]. The macronodular form is extremely rare [4,7]. The imaging type in the present case was of the uncommon macronodular form not associated with splenomegaly.
